Assessment Survey Script & Resources

Use the directions below for the before and after Dream Path SEL Assessment Survey in Lessons 1 and 10.

  1. Go to dreamline.org/pre-survey1 (for Lesson 1) 

    You can email the link, drop in chat, or post on your class site.

  2. Use the QR code or the web address in their Dream Path Journal

PREPARATION:

  1. Read over the Student Direction Page at dreamline.org/pre-survey1.

  2. Take a list of your students and assign each one a number — or use existing student numbers if you have them. Keep the list for use at the END of the Dream Path Program when they take the assessment again (to measure change.)

  3. Look through the content and form of the survey as well as the directions below before you give it to students.

  4. Check to make sure the Google Form is accessible to students from school computers. (This is a precaution to avoid wasted time.

  5. Arrange the time and place where students can take the survey. This must be under your direct supervision, though it can be done remotely if you are on live Zoom with students.  

ADMINISTERING THE SURVEY: 

  1. Give students the link for the Survey and instruct them in the following manner.

    1. Go over directions on the linked page with students. Answer any questions. 

    2. Make sure each student knows what their student number is. 

    3. When they are ready, have them click the Begin Survey button.  

    4. We will fill out the About Section now together. 

      1. For number 1 select your age from the dropdown. Let me know when you’ve done that. (Thumbs Up or the like)

      2. For number 2  in our school name. (Write on the board or tell them how to spell it as needed.) 

      3. For number three write ________ (Provide the Group Name you used when you registered for the Dreamline Mobile App)l 

      4. For number four write your student number.

    5. Now we’re ready for the survey section. For each statement, choose the response that most closely matches how you feel about it today. Respond to every item as  honestly as you can. There are no correct or incorrect answers to these questions. Use the pictures to choose the right number for you. Do you have any questions? (answer any questions)

    6. OK. Please begin the survey. Answer all of the questions. When you get to the end. remember to click the SUBMIT button to complete the survey. 

    7. If you have a question about what something means you may ask me, but you must choose your replies on your own. Just answer honestly based on how you feel right now.
